California Healthy Kids Survey
Your child is being asked to take the California Healthy Kids Survey this year. The survey includes questions about alcohol, tobacco, and other drug issues, as well as questions about school safety and the learning environment. Student participation in the survey is voluntary. The survey is anonymous. No names or any other identifying information is connected to the answers except for the name of the school.
All 6th grade students will need active consent to complete the survey. All 7th, 9th and 11th grade students will complete the survey unless a Withdrawal Form is submitted to your child’s school office by April 20th.
We hope that you will give permission for your child to participate in this process so that his or her opinion and knowledge can be used to help us make improvements within our school.
